Mentor Agreement Form
Welcome to the NNCAUK Mentorship Academy! This agreement ensures clarity and alignment of expectations between you and NNCAUK. Please read the terms carefully before signing.
Mentor Roles and Responsibilities
Provide professional guidance, support, and mentorship to assigned mentees in alignment with the goals of the NNCAUK Mentorship Academy.
Commit to a minimum of 2 hours per month for mentorship sessions, which may include one-to-one meetings, group workshops, or other agreed-upon activities.
Maintain confidentiality regarding all information shared by mentees during mentorship sessions.
Act professionally and respectfully in all interactions with mentees, NNCAUK staff, and other participants.
Notify NNCAUK immediately if they are unable to continue their mentorship duties or if any conflicts of interest arise.
Provide feedback on the mentorship programme when requested, including completing any evaluation forms.
Adhere to the NNCAUK Mentorship Academy's Code of Conduct and Terms and Conditions.
NNCAUK's Responsibilities
Provide mentors with the necessary resources, tools, and support to effectively carry out their mentorship duties.
Match mentors with mentees based on their expertise, availability, and the mentee's goals and preferences.
Ensure that all mentorship sessions are conducted in a safe, respectful, and professional environment.
Address any concerns or disputes that may arise during the mentorship programme in a timely and fair manner.
Recognise mentors for their contributions, including providing certificates of participation and opportunities for professional development.
Confidentiality and Data Protection
Mentors must maintain strict confidentiality regarding all information shared by mentees during mentorship sessions.
Mentors must not disclose any personal or professional information about mentees to third parties without prior written consent.
NNCAUK complies with GDPR and other data protection regulations. Personal data collected during the mentorship programme will only be used for programme-related purposes.
Any breach of confidentiality may result in the termination of the mentor's participation in the programme.
Attendance and Availability
Mentors must commit to attending all scheduled mentorship sessions and notify mentees in advance if they are unable to attend.
Mentors must provide at least 48 hours' notice if they need to reschedule a session.
Mentors must update NNCAUK if their availability changes during the programme.
Termination of Agreement
NNCAUK reserves the right to terminate this agreement if the mentor:
Breaches the Code of Conduct or Terms and Conditions.
Fails to meet their responsibilities as outlined in this agreement.
Engages in unprofessional or inappropriate behaviour.
Mentors may voluntarily withdraw from the programme by providing written notice to NNCAUK.
Upon termination, mentors must return any NNCAUK resources or materials in their possession.
Liability
NNCAUK is not liable for any personal or professional outcomes resulting from participation in the mentorship programme.
Mentors are responsible for ensuring they have the necessary equipment and internet access for online sessions.
NNCAUK is not responsible for any technical issues or disruptions during online sessions.
